The verdict
Twelve tours, four hundred dollars between the cheapest and the dearest, and itineraries that are far more alike than the prices suggest. Here is a straight recommendation for each kind of visitor, with the reasoning attached.

Quick answer
For most visitors, the day that includes Kegon Falls and Lake Chuzenji as well as Toshogu, $74 — 2,272 reviews, more than any other Nikko listing. For a party of three or four, the private guide at $66 is the anomaly worth knowing about: it costs less than most coach seats. For a private day with real evidence behind it, $311 at 4.9 across 316 reviews.

The reasoning
If you disagree with a verdict, this is the part to argue with.
2,272 reviews is more evidence than every other Nikko listing on this page combined, the lake is genuinely included rather than mentioned, and at $74 it is not the expensive option.
At $66 it is the cheapest listing here and it is not a coach. Forty-five reviews at 5.0 is a young listing rather than a doubtful one, and the pricing is the most interesting anomaly on the page.
Toshogu and Kegon Falls at $83 with 1,506 reviews. It does not reach the lake, and for a lot of visitors the waterfall alone is enough of the mountains.
All three UNESCO sites at $88 and rated 4.7, the best of the coach days. It trades the mountains for Futarasan and Rinnoji, which is a real choice rather than a compromise.
4.9 from 316 reviews is the best-evidenced private listing here by a wide margin, and the route is yours.
It is the only listing that ends in hot water, which is what Nikko was used for long before the coaches. One review, so read it as an itinerary rather than a rating.
Whatever you book, the lake column is the one thing to read twice.

By weight of evidence, the lake day at 4.6 from 2,272 ratings. By score, the private full day at 5.0 from 436, and the personal private day at 4.9 from 316.
$66, and it is a private guide rather than a coach seat — which is the oddest thing about the pricing on this page and worth knowing before you book anything.
For three or four people, often yes. Several private listings are priced per vehicle or per guide rather than per head, so the arithmetic changes sharply above two travellers.
If the mountains interest you at all, yes. The lake and the falls are the half of the day most visitors did not know was coming, and several tours leave them out without saying so clearly.
